Confidentiality and
Access to Records
Confidential patient data will be shared within the practice health
care team and with other health care professionals.
Confidential and identifiable patient information will not be
disclosed otherwise without explicit consent, unless:
It is a matter of life and death or serious harm to you or to
another individual. It is overwhelmingly in the public interest to do so. There is a legal obligation to do so.
In all of these circumstances the minimum identifiable information
that is essential to serve the purpose may be revealed to someone
with a legal entitlement to access the data for that purpose.
If you are concerned about any issues regarding your confidential
data, further information is available from the practice manager.
You are entitled to register an objection, which will be respected
if this is possible.
Patients have a right to access their own health records and obtain
a copy of their records, view their records or have information
explained where necessary.
All requests must be made in writing, and there may be a charge
to have a printed copy of your notes. Please ask at Reception for an
Application for Access to Medical Records form.
Your request will be actioned as soon as possible, and within a
maximum of 21 days.
